Hei ja tervetuloa blogiini!



Olen 22-vuotias naisenalku Turusta. Olen opiskellut yliopistossa nyt kaksi vuotta, ja olen töissä kaupassa opintojen ohessa. Blogini tarkoitus on jakaa mietteitäni ja arkeani kroonisen sairauden, servikaalisen dystonian, kanssa elämisestä. Aion kirjoittaa itseäni askarruttavista asioista sairauden kanssa elämiseen, ja yleensäkin elämääni ja tulevaisuudensuunnitelmiini liittyen. 


Ajatus blogin pitämisestä sairauteen liittyen on pyörinyt mielessäni jo jonkun aikaa. Tänään olo tuntui tarpeeksi rohkealta kokeillakseni sitä ihan oikeasti. Uskon että mietteiden kirjoittaminen ja jakaminen voi toimia itselleni tavallaan kuin terapiahoitona koska askarruttavia asioita todellakin tällä hetkellä on paljon. Toivon myös että voin blogini avulla lisätä tietoisuutta dystoniasta, kertomalla oman kokemukseni kautta miten se on tähän mennessä omaan elämääni vaikuttanut. Ehkä blogiini eksyy myös muita kohtalotovereita, ja erityisesti muihin samanikäisiin dystoniaa sairastaviin olisi mukava tutustua. Kuitenkin kaikenikäiset ja muutenkin kaikenlaiset ihmiset ovat erittäin tervetulleita lukemaan minun juttujani ja kommentoimaan, jos vaan kiinnostusta löytyy :)


Näin aluksi liitän tähän pienen kuvauksen dystoniasta, jos tästä sairaudesta ei ole aikaisemmin kuullut. Suomen Dystonia-yhdistyksen kuvaus sairaudesta on seuraavanlainen: "Dystonia on neurologinen liikehäiriö, jossa tahdonalaisten lihasten liikkeitä säätelevien osien toiminta aivoissa on häiriintynyt. Häiriö voi kohdistua mihin tahansa tahdonalaiseen lihakseen tai lihasryhmään. [...] Dystonia, jonka oireet ovat yksilöllisiä, aiheuttaa lisääntynyttä lihasjännitystä, virheasentoja tai lihasten tahdosta riippumattomia, toistuvia ja nykiviä liikkeitä ja kipuja. [...] Syytä dystonian syntyyn ei vielä tiedetä. [...] Oireita voidaan lievittää liiallisesti toimivaan lihakseen pistettävällä botuliinitoksiinilla. Hoidon tukena käytetään myös lihaksia rentouttavaa tai kipuja lievittävää lääkitystä. Parantavaa lääkehoitoa ei ole. Oireita ja niiden aiheuttamaa liikuntaelimistön epätasapainoa voidaan lievittää myös fysioterapian ja liikunnan avulla." (lähde: http://www.dystoniayhdistys.com)

Itselläni oireet alkoivat noin vuosi sitten, eli vuoden 2015 alkusyksystä. Kävin syksyllä fysioterapeutilla työterveyden kautta hakemassa apua ajoittaisiin niska-hartia kipuihin. Fysioterapeutin mielestä asennossani ja ryhdissäni oli virheitä, joten hän epäili kipujen johtuvan siitä. Hän kehoitti tekemään harjoitteita joiden avulla voisin korjata ryhtiäni ja erityisesti pääni etukenoa asentoa. Tehtyäni näitä harjoitteita muistaakseni noin viikon aloin huomata outoja kipuja, jotka olivat voimakkaampia kuin ennen. Ensimmäinen huomattava oire oli jatkuva pääkipu ja hartioiden ja niska-alueen lihasten kireys. Olin ennenkin kärsinyt niskakivuista ja pääkivusta, mutta ne yleensä helpottivat särkylääkkeillä, levolla ja venyttelyllä. Tällä kertaa oireet eivät vain loppuneetkaan. Söin särkylääkkeitä kivun lopettamiseski, mutta isommat annoksetkaan eivät vieneet kipua kokonaan pois. 


Loppusyksystä mukaan tulivat myös migreenikohtaukset, jotka saattoivat viedä sängynpohjalle koko päiväksi. Kaikki oireet olivat yleensä lievempiä aamulla ja aamupäivästä, ja kipu yltyi pahaksi yleensä vasta iltapäivällä. Fyysinen rasitus töissä, urheilu sekä myöskin opiskelu tuntuivat pahentavan oloa ja tämän takia kävinkin vielä loppusyksyn ja talven aikana kolme kertaa lisää fysioterapeutilla ja kaksi kertaa yleislääkärin vastaanotolla. Fysioterapeutit ja lääkärit olivat suhteellisen yksimielisiä siitä mikä minua vaivasi, ja sanoivat oireiden johtuvan selkä- ja kaularangan virheellisestä asennosta, joka taas ylikuormittaa selän ja niskan lihaksia aiheuttaen jännityspäänsärkyä. Maaliskuuhun mennessä oireet eivät olleen kuitenkaan helpottaneet, ja migreeni oli tullut lähes päivittäiseski seuraksi lisääntyneiden selkä- ja  niskakipujen kanssa. 


Maaliskuun alussa äitini sai minut suostuteltua varaamaan ajan neurologilta, ja siellä käytyäni sain reseptin migreenin kohtauslääkitykseen, ja kehotuksen jatkaa triptyl-lääkkeen syöntiä, jonka yleislääkäri oli minulle määrännyt noin kuukausi aikaisemmin niskasärkyjen lopettamiseski. Tämän jälkeen migreenit hieman hellittivät, mutta taustalle jäi kuitenkin jatkuva päänsärky ja niska-selkäkivut. Kävin siis noin 6 viikon päästä uudestaan samalla neurologilla, joka osasi epäillä dystoniaa kipuoireiden ja pääni asennon perusteella. Hän oli kuitenkin suhteellisen varma ettei minulla dystoniaa olisi, koska pään asento oli kuitenkin suhteellisen normaali eikä vääntänyt kummallekkaan sivulle. Lääkäri ohjasi minut varaamaan ajan toisele neurologille joka oli erikoistunut dystoniaan ja sen hoitoon. 

Toukokuun alussa menin tämän toisen neurologin vastaanotolle, ja kerrottuani oireeni hän tutki pääni asentoa ja liikkuvuutta. Hän sanoi että on hyvin epätodennäköistä että minulta tätä lihasten yliaktiivisuutta löytyy, mutta tekee tutkimuksen jos haluan. Olin jo ollut niin kauan kipeä, joten suostuin ENMG-tutkimukseen. Tässä mittauksessa kävi heti selväksi meidän molempien yllätykseksi että minulla tosiaan löytyi paljonkin yliaktiivisuutta sekä niskan, kaulan että selän lihaksista. Sain diagnoosiksi "servikaalinen dystonia" ja lääkäri määräsi minulle botuliinipistoshoitoa, ja kävin ensimmäisen kerran piikillä kesäkuun puolessa välissä. Koska yliaktiivisuutta löytyi aika laajalta alalta (kaula. niska, hartiat, selkä ( ja purentalihakset luultavasti)) ja ensimmäinen pistetty annos oli vain 100 yksikköä, monet alueet jäivät nyt vielä ensimmäisellä kerralla hoitamatta.


Nyt pistoksien antamisesta on kulunut kolme viikkoa, ja odottelen vieläkin helpotusta oireisiini. Olen tällä hetkellä sairaslomalla töistä koska fyysinen rasitus pahentaa oireita ja en yksinkertaisesti koe pystyväni tekemään työtä tällä hetkellä kipujen vuoksi. Kesän aikana oireet ovat vain pahentuneet, ja kipujen lisäksi mukaan on tullut käsien vapinaa jos lihakset ovat väsyneet, muutenkin yleistynyttä lihasheikkoutta ja ajoittaisia lihaskramppeja jaloissa ja käsivarsissa. Onneksi joskus on myös lähes tai kokonaan kivuttomia päiviä, ja ihan lähipäivinä olo on ollut jo parempi. Joten toivo siis elää että lääke rupeaa pikkuhiljaa oireitani lievittämään, ja kivuttomia päiviä tulee lisää. 


Nyt kun tämä sairaskertomus on saatu pois alta, voin ensi kerralla kirjoitella jostakin hieman yksityiskohtaisemmasta aiheesta. Toivttavasti edes joku jaksoi lukea loppuun, kommentoida ja kysellä saa ihan rohkeasti jos haluaa.

I'm a 22-year-old woman from Turku, Finland. I study at the university and work part time in a small grocery store. I am diagnosed with a chronic brain disorder, Cervical Dystonia, and in this blog I will share how I cope with the disorder and how it affects my life.

Here is a brief explanation of Dystonia to anyone who hasn't heard about it:
"Dystonia is a disorder characterized by involuntary muscle contractions that cause slow repetitive movements or abnormal postures. The movements may be painful, and some individuals with dystonia may have a tremor or other neurologic features. There are several different forms of dystonia that may affect only one muscle, groups of muscles, or muscles throughout the body. Some forms of dystonia are genetic but the cause for the majority of cases is not known.
Dystonia can affect many different parts of the body, and the symptoms are different depending upon the form of dystonia. Early symptoms may include a foot cramp or a tendency for one foot to turn or drag—either sporadically or after running or walking some distance—or a worsening in handwriting after writing several lines.  In other instances, the neck may turn or pull involuntarily, especially when the person is tired or under stress. Sometimes both eyes might blink rapidly and uncontrollably; other times, spasms will cause the eyes to close.  Symptoms may also include tremor or difficulties speaking.  In some cases, dystonia can affect only one specific action, while allowing others to occur unimpeded.  For example, a musician may have dystonia when using her hand to play an instrument, but not when using the same hand to type.  The initial symptoms can be very mild and may be noticeable only after prolonged exertion, stress, or fatigue.  Over a period of time, the symptoms may become more noticeable or widespread; sometimes, however, there is little or no progression. Dystonia typically is not associated with problems thinking or understanding, but depression and anxiety may be present." Source: http://www.ninds.nih.gov)

I started having symptoms of Dystonia about a year ago, in early fall of 2015. I went to see a physiotherapist because I was experiencing some neck and back pains that were distracting me from my studying and working. The neck and back pains were not concerning me too much though, because I was very used to having mild pains caused by studying or working. The physiotherapist said that I have some issues with my posture, so the pain could be because of it. She gave me some exercises to do at home to improve my posture, especially my forward slanted neck. After doing these exercises about a week or so, I started experiencing strange pain that were a lot stronger than the ones I had experienced prior to the physiotherapist visit. The first noticeable symptom was a constant headache and I also experienced that my neck and shoulder muscles got a lot more tense and tight. Like I said, I had had these neck and shoulder pains before, but usually they healed with painkillers, rest and stretching. However, this time none of that worked. I ate a lot of painkillers (probably too much and for too long) to numb the pain, but this time somehow they didn't work. 

In late fall of 2015, I started experiencing migraine attacks that sometimes lasted for days. Most days though, the pain was more manageable in the morning, but during the afternoon it got a lot worse. All kind of physical work, exercising, and even sitting for longer periods of time when studying made the symptoms worse. I went to see a physiotherapist three more times in the next few months and I also went to see a general practitioner two times to get some help. All of them seemed to be quite unanimous about what was the problem with me: They said that I have issues with my posture which creates too much pressure to my muscles in my neck and back which then again causes headache and migraines. I was told to keep doing different kinds of exercises to improve my posture, and I was prescribed triptyl (to help control chronic muscle tension and pain). However, none of that helped, and in March 2016 I was suffering from daily migraines and my neck and back pains had gotten a lot worse. It was difficult for me to keep up with my studying and work at the same time and I was so exhausted and depressed because of the constant pain. 

Therefore, in March I went to see a neurologist. He prescribed me with new medication to treat my migraines, that proved to be somewhat helpful. In the next couple of months, I stopped having so many migraine attacks, but I was still left with constant headache along with neck and back pains. After about 6 weeks from my last visit to the neurologist, I went to see him again. This time, after examining me, he suspected I might have a disease called dystonia. I remember him saying that it was very unlikely, because my head didn't turn or twist to the side, it was just slightly in a onward position. At this point I didn't experience any other major symptoms of dystonia, besides pain and muscle tension. Luckily the doctor still suggested that I should see another neurologist who was specialized in treating dystonia patients. 

In May 2016 I went to see this other neurologist. After she had examined me, just like the previous doctor, she said it was very unlikely that I have dystonia but if I wanted to, she would examine my neck and back muscles with ENMG machine. I agreed to it, since I had been sick for so long and really, I had nothing to lose. We were both very surprised when it turned out that I actually have the disease. I was diagnosed with Cervical Dystonia, and she found the disease in my neck, shoulders and back. In June, I went to get my first Botox injections. Since I have the disease in many areas of my body, (front and back of the neck, shoulders, back (and probably my chewing muscles)) the first dose of botox wasn't enough to treat all areas. 

Now, it has been three weeks since the injections and I'm still waiting for the pain to ease. During the summer, my symptoms have only gotten worse and I've also started to experience other symptoms, such as tremors in my hands, muscle weakness and cramps in my arms and legs. Luckily, sometimes I have days that I'm almost painless and just in the few last days I've been feeling a little bit better. So there is hope that the injections work for me.

I apologize if I misspelled  some medical terms as I yet barely know them in Finnish, but I hope you got the basic idea here.
